Hemorrhoids are enlarged or varicose veins of the tissues if the abuser rectal outlet. The are the most frequent cause of rectal bleeding. Anal and perianal pruritus(itching), soreness and excoriation occur commonly in patients suffering from hemorrhoids, fistulas and proctitis.
Careful local toilet with attention to any minor fecal soiling, adjustment of diet to avoid hard stools, the use of bulk-forming materials such as bran and a high residue diet are helpful.
Soothing preparations containing mild astringent such as bismuth subagallate, zinc oxide and hamamelis with lubricants, vasoconstrictors or mild antiseptics, in the form of topical ointments, creams and suppositories, are used to provide symptomatic relief. Local anaesthestics are included are included in some preparations to relieve pain e.g lidocaine gel.
Corticosteriods may be combined in such preparations (but should only be used after exclusion of infection); they are suitable for occasional short -term use, but prolonged use may cause atrophy (thinning) of the anal skin.
Anal and perianal itching, soreness and excoriation occur commonly in patients suffering from hemorrhoids, fistulas and proctitis . These conditions are best treated by application of bland ointments, suppositories and dusting-powders. Adjustment of diets to avoid hard stools is also helpful.

Vasoconstrictors such as phenylephrine may also be included in the preparations for hemorrhoids.
Vasoprotectives such as Diosmin are used in treating hemorrhoids. Diosmin plus hesperidin produces Daflon. Daflon tablets are now very useful in the treatment of hemorrhoids.. it helps promote venous return, shrinks the dilated and engorged veins of the piles with associated symptomatic relief of pains. It comes as 500mg tablets.
Local Anaesthetics are used to relieve pain associated with hemorrhoids, and pruritis ani(anal itching). They include lignocaine and pramoxine. They should not be used for longer than 2 weeks since they may cause sensitization of the anal skin.
Corticosteroids such as Betamethasone and hydrocortisone are often combined with antibiotics, local anaesthetics , and soothing agents for short term use in piles but not in the presence of herpes infection.
Administration: suppository should be inserted into the rectum night and morning and after a bowel movement.
Rectal ointments and creams are applied night and morning after a bowel movement, externally or by rectum using a rectal nozzle.
