An ovarian cyst is a fluid-filled sac that forms in the ovary. Ovarian cysts are common and, in the vast majority of cases, they are benign (non-cancerous) in patients younger than 35.
Ovarian cysts affect fertility if they interfere with normal ovulation or represent a mechanical obstacle for the fertilisation proces.
Can ovarian cyst affect fertility?
Generally, in some cases we know that there are some types of ovarian cysts that are associated with decreased fertility. What we need to know and decided on is what type of ovarian cyst you have.
There are two types of cyst that can affect your fertility include:
1. Endometriomas. These are cysts caused by endometriosis, a condition in which the tissue normally lining your uterus (endometrium) grows outside the uterus. Endometriosis can occur any where in the body, but we they occur on the ovaries they are called Endometriomas which can be associated with fertility problems.
2. Polycystic ovary syndrome: Ovarian cysts resulting from polycystic ovary syndrome.
Polycystic ovary syndrome (PCOS) is a condition which is characterised by numerous small cysts within the ovaries marked.
PCOS is associated with irregular ovulation which leads to irregular periods. Irregular periods is associated with infertility in some women.
We also have some ovarian cysts which are not associated with fertility:
1. Functional cysts. Functional cysts are normal and they occur with and during a normal menstrual cycle. They are the most common type of ovarian cysts and don’t cause or contribute to infertility. They are related to the normal functioning of your cycle, and often mean that you are fertile.
2. Cystadenomas. These are usually found on the surface of the ovary. They are not known to cause or associated with infertility.
3. Dermoid cysts. These solid cysts contain tissue — such as skin, hair or even teeth — instead of fluid. Dermoid cysts aren’t associated with infertility.
Symptoms of ovarian cyst
Most ovarian cysts are small and don’t cause any problems. When there are symptoms, you might have pressure, bloating, swelling, or pain in one side of your lower belly. This pain may be sharp or dull, and it can come and go.
Ovarian cyst treatment
Most cysts go away on their own without any medical intervention. Your doctor may suggest watching and waiting for changes.
Your doctor can give you medicine for pain like ibuprofen, paracetamol and even tramadol depending on your pain level. They might also prescribe birth control pills. The hormones in the pills won’t make the cysts go away, but they can help prevent new ones.
Some ovarian cysts need surgery. That includes those that are large, don’t go away, or cause symptoms due to ovarian torsion. You could also need surgery if you’re near menopause, because cysts are more likely to be cancerous. Depending on your case, the surgeon may take just the cyst or the entire ovary.
There are different types of surgery:
Laparoscopy is for smaller cysts. The doctor makes a tiny cut above or below your bellybutton. A small tool with a camera allows your doctor to see inside, and a different tool removes the cyst or ovary. You probably won’t have to stay in the hospital overnight. Laparoscopic surgery for ovarian cyst removal is quite expensive.
Laparotomy is for cysts that may be cancerous. It is done with a bigger cut in the abdomen.

Most ovarian cyst removal surgeries in Nigeria are not laparoscopic (keyhole surgery) but rather involves an open abdominal surgery. The cost of ovarian cyst surgery in Nigeria is around #200,000 – #500,000 Naira or more depending on the hospital, city, surgeon and even the patient’s clinical state. In Lagos, Abuja, Port Harcourt, and Benin city for instance, ovarian cyst surgery in private hospitals like Lily hospitals, Cedacrest, St.Nicholas, Lagoon Hospital etc, is around 5 million Naira or even more.
